The Re-Engineered Discharge for Diabetes Care Transitions (REDDCAT2): Screening and Addressing SDOH needs at hospital discharge

1.Dr. Carlozzi, site principal investigator, will provide input into the scientific direction of the study throughout the life of the project. She will be involved in making scientific decisions, particularly with regard to interpretation and analysis of the patient-reported outcome data, in collaboration with Dr. Mitchell, Dr. Bruce Barton, and the investigative team. 2.U-M will be responsible for the designing and programming the survey data collection platform and developing training for the end user. They will provide database support throughout the study and will implement any changes that are needed to the database throughout the duration of the study. 3.U-M will be responsible for assisting with the primary statistical analyses for this study (included the analyses related to the experimental study aim) and for directing and conducting secondary and tertiary analyses related to patient-reported outcome measures that will be collected as a part of this study. 4.Site personnel will attend regular team meetings. 5.Site personnel will assist with study dissemination efforts including manuscript writing, poster presentations, etc.
